Diagnostic prediction models to identify patients at risk for healthcare-facility–onset Clostridioides difficile: A systematic review of methodology and reporting

Objective:
To systematically review the methodology, performance, and generalizability of diagnostic models for predicting the risk of healthcare-facility–onset (HO) Clostridioides difficile infection (CDI) in adult hospital inpatients (aged ≥18 years).
Background:CDI is the most common cause of healthcare-associated diarrhea. Prediction models that identify inpatients at risk of HO-CDI have been published; however, the quality and utility of these models remain uncertain.
Methods:Two independent reviewers evaluated articles describing the development and/or validation of multivariable HO-CDI diagnostic models in an inpatient setting. All publication dates, languages, and study designs were considered. Model details (eg, sample size and source, outcome, and performance) were extracted from the selected studies based on the CHARMS checklist. The risk of bias was further assessed using PROBAST.
Results:Of the 3,030 records evaluated, 11 were eligible for final analysis, which described 12 diagnostic models. Most studies clearly identified the predictors and outcomes but did not report how missing data were handled. The most frequent predictors across all models were advanced age, receipt of high-risk antibiotics, history of hospitalization, and history of CDI. All studies reported the area under the receiver operating characteristic curve (AUROC) as a measure of discriminatory ability. However, only 3 studies reported the model calibration results, and only 2 studies were externally validated. All of the studies had a high risk of bias.
Conclusion:The studies varied in their ability to predict the risk of HO-CDI. Future models will benefit from the validation on a prospective external cohort to maximize external validity.

At the time of writing, work continues on a replacement for the Clergy Discipline Measure 2003 (CDM 2003). This comment explores some issues which have arisen in a recent disciplinary case – Re Evans – where, for the first time, the boundaries of the CDM jurisdiction have been considered by the tribunal. I will first identify the salient facts of the Evans case, before moving on to explore the specific issue of jurisdiction. I conclude with some observations about why this case is significant, especially for those working on the replacement to the CDM 2003.

A 7.6-m core recovered from Lough Inchiquin, western Ireland provides evidence for rapid and long-term climate change from the Late Glacial period to the Mid-Holocene. We determined percentage of carbonate, total organic matter, mineralogy, and δ18Ocalcite values to provide the first high-resolution record of climate variability for this period in Ireland. Following deglaciation, rapid climate amelioration preceded large increases in GISP2 δ18Oice values by ∼2300 yr. The Oldest Dryas (15,100 to 14,500 cal yr B.P.) Late Glacial event is documented in this record as a decrease in δ18Ocalcite values. Brief warming at ∼12,700 cal yr B.P. was followed by characteristic Younger Dryas cold and dry climate conditions. A rapid increase in δ18Ocalcite values at ∼10,500 cal yr B.P. marked the onset of Boreal warming in western Ireland. The 8200 cal yr B.P. event is represented by a brief cooling in our record. Prior to general warming, a larger and previously undescribed climate anomaly between 7300 and 6700 cal yr B.P. is characterized by low δ18Ocalcite values with high-frequency variability.

Tape Casting Technique for Fabrication of Graded-Density Impactors for Tailored Dynamic Compression

Recently, the use of graded density impactors for dynamic compression experiments has received increasing interest. These gas gun experiments have demonstrated complex loading paths which can last microseconds, and may be capable of bridging the timescales of existing static and dynamic compression experiments. A tape casting technique has been developed for fabrication of the impactors. In the technique, a series of tapes were prepared in the Mg-Cu system with compositions ranging from 100% Mg to 100% Cu. The tapes were characterized for their layer thickness, density, and sound wave velocity. Impactors were fabricated by punching individual layers from the tapes, stacking and laminating them, removing the organic matrix, and hot-pressing the laminated structure. The density profile is determined by the order in which the tapes are stacked in the laminate and is therefore highly flexible. The resultant thickness and average density of the impactors is consistent with the data for the individual layers. Impactors were characterized for uniformity by ultrasonic C-scan and white light interferometry. Dynamic compression experiments were performed on a two-stage helium gas gun using the graded density impactors. Results will be presented and shown to agree well with hydrocode modeling.
